Book excerpt: Abstract: This paper explores the components of a patient centered medical home as it relates to children and youth with special healthcare needs. A survey from the State and Local Area Integrated Telephone Survey (SLAITS) will be used to explore if and how the components of a Patient-Centered Medical Home (PCMH) are embodied in current medical practice in ambulatory care settings across the United States. This survey included multiple questions that would indicate whether or not their clinicians' practices involved continuity of care to increase their effectiveness. Using Pearson's correlation and chi-square analysis, it was concluded that most families surveyed by SLAITS were in fact receiving coordinated care that defined various components of the patient-centered medical home.
